The Chief Minister’s Comprehensive Health Insurance Scheme (CMCHIS) is a program in Tamil Nadu which was launched in the year 2009. This health insuance scheme provides expenses of medical treatment under Government and private hospitals. It is a scheme for people who are below poverty line (BPL) and who cannot afford costly treatment. A family whose annual income is less than INR 1 lakh can per annum can avail the benefits of the CMCHIS card. Eligibility Criteria to Apply For CMCHIS

For an individual to apply for the Amma Kapitu Thittam Card, they must be a resident of Tamil Nadu and should earn less than a set threshold.

1. Residency

Only residents of Tamil Nadu are eligible for this scheme. Residency is verified using an Aadhaar card, ration card, or voter ID card submitted during the application process.

2. Annual Income

Applicants must have an annual income of ₹72,000 or less, as per the CMCHIS guidelines. An income certificate stating this must be provided.

3. Family Members

Eligible beneficiaries include the applicant, their legal spouse, unmarried children under the age of 25, and dependent parents living with the applicant.

Documents Required

To register for CMCHIS, you must also submit the following documents. Failure to do so may render you ineligible.

  • Aadhaar card for each family member or any equivalent government-issued ID.
  • A ration card to confirm family structure and members.
  • A self-declaration form that must be filled in and signed by the head of the family.
  • Bank account details are needed to link the card and transfer benefits.

1. Obtain an Income Certificate

Secure an income certificate proving that your annual income is below the prescribed limit. This can be obtained from the revenue authority or administrative officer of your village.

2. Prepare Required Documents

Ensure you have the following documents ready:

  • Original ration card and a photocopy.
  • Signed self-declaration form.
  • ID proof with a photo, such as an Aadhaar card.
  • Residential proof.
  • PAN card.

3. Visit an Enrollment Center

Take all the required documents to the nearest enrollment center and present them at the kiosk. The authorized person at the kiosk will verify your documents.

4. Biometric Verification

After your documents are verified, biometric scanning will be conducted. Upon successfully completing document and biometric verification, your card will be  issued.

How to Download the Amma Kapitu Thittam Card?

Downloading the Amma Kapitu Thittam Card is a straightforward process. Follow these simple steps to download your card.

Step 1: Visit the Official Website

Open Google and type https://www.cmchistn.com/, then hit Enter. This will redirect you to the official website of the Chief Minister's Health Insurance Scheme (Tamil Nadu).

Step 2: Locate the E-Card Option

Look for an option that states "Generate E-Card" or a similar link to generate your health insurance card.

Step 3: Provide KYC Details

Fill in the required KYC information, including your name, date of birth or age, family details, and Aadhaar card number. Ensure that all details are entered accurately.

Step 4: Generate the E-Card

Click on the "Generate E-Card" button. After your information is verified, the card will be generated and displayed on your screen.

Step 5: Save and Print

Verify that the details on the card are accurate. Save the card to your computer's hard drive and print it as needed.

Note: 

  • The physical CMCHIS card will be sent to your residence address. 
  • The e-card or printout is a valid substitute for your physical card should you lose it.
