- Step 4:- An e-card will be provided after the verification process has concluded.
How to Download the Chief Minister Health Insurance E-Card
An e-card is issued during the enrollment for the CM Health Insurance Card. After the issuance of the card or even in case the member has misplaced their card, they can easily download their card online. The steps for downloading the card online are as follows:-
- Visit the website page of the Chief Minister Health Insurance
- Click the Smart Card Download button
- Enter the beneficiary’s details
- Enter the URN/KYC details along with the ration card number
- Click on ‘Generate e-card’
- Download the e-card.
After these steps are completed the member will have their new e-card available to them. To download the new card the member has to end the process. However, in case of medical emergencies the resident can use the downloaded copy of this government health insurance scheme.
Documents Required for Enrollment in CMCHIS
The following documents are required by the citizen during the process of enrollment for the Chief Minister Comparative Health Insurance Scheme:-
- Official Income Certificate
- Family Card
- Aadhar Card
- Self-declaration by the head of the family or the family member.
- Original Ration Card and one photocopy of the original ration card.
- Address and identity proof such as the Aadhar card or the PAN card.
